    The Activities Fair is one of the best ways for clubs to gain exposure and develop membership. To register and request a table, please use the form below. Only one form should be submitted for each club, so please coordinate with other executive board members.

  • Student Organization Concern

    This form should be utilized when there is a concern or a potential policy violation in regards to a student organization. This form submission will be reviewed by the leadership of the Student's Association Appropriations Committee and Administration & Review Committee to determine the next steps and a timeline.

    Need access to a Student Life Space? You can request swipe access to several spaces around campus or get access to your organization's storage location.

  • Supplemental Funding Request

    The College Supplemental Fund Application allows undergraduate students in AS&E to request supplemental funding as an individual or on behalf of a group to participate in academic, professional, or experiential programs. Funding is available on a rolling basis.

  • SWARM Training Request

    SWARM (Students Wanting Alcohol Responsibly Monitored) Event Management training is designed for student leaders hosting events that include alcohol. SWARM Risk Reduction training was created for student leaders that want to promote safe drinking habits on campus. Please register two weeks before the date you’re requesting. Email Ashley Toland with questions or if you want to register a training for less than 5 people.

  • Accident/Injury Form

    This form must be submitted within twenty-four (24) hours of an accident.

  • Accolades and Awards Submission

    This form is a way to share exciting news about you, a student organization, or another member of a student organization. Wilson Commons Student Activities will feature the exciting and noteworthy news through their outlets. Some past examples are: On January 20 at Georgetown University, Mock Trial came in fourth out of twenty-four nationally ranked teams, with a record of 6-2. Jonathan Dietz & Hannah Brennan each won Outstanding Witness Awards.

  • CCC Support Request

    If you notice issues with the main CCC site or the Campus Groups app, please submit a support request to report the bug or get troubleshooting assistance.

  • Community Kitchen Training Sign-Up

    Want to host an event in one Wilson Commons' fully stocked kitchens? You can sign up for an upcoming training using this form. For more information, check out the Community Kitchen information page or contact Ken Beck, assistant director of student life operations.

  • Gender Exclusivity Waiver

    Affiliated organizations whose existence or activities depend upon following gender-exclusive membership policies set by an external governing body, such as a national organization or intercollegiate league, or otherwise believe that gender-exclusive membership policies are warranted must apply for a gender exclusivity waiver.

  • Holiday Shopping Fair Vendor Application

    Our Holiday Shopping Fair is an annual tradition that WCSA has hosted for over ten years. Traditionally held at the begining of December, the fair is a way for students, faculty, and staff from across the University to interact and do some holiday shopping.   • Table Request for Off-Campus Vendor

    This form allows non-University affiliates the opportunity to advertise or sell on campus. All vendors are required to follow the University policies and tabling policies.

  • Election Violation Report

    This form should be used to report potential campaign violations. Not all reports will result in penalty points to candidates. You will receive a response to your report within 48 hours. Campaigning rules are available here.

  • Fall Election Candidacy Submission

    This form will allow you to declare your interest in Senate, Class Council, and/or Hall Council. More information about Fall 2019 Elections can be found on the Students' Association Elections website. Visit the Students' Association Election Website.

  • Accident and Injury Form

    This form must be submitted within twenty-four (24) hours of an accident.

  • Individual Travel Waiver and Release

    In order to travel, this form must be completed and approved by 3 p.m. the business day prior to departure. For example, if traveling on a Saturday or Sunday, this form must be completed and approved by 3 p.m. Friday. Typical processing time is one business day.

  • Group Travel

    The Group Travel form must be completed by the trip manager at least one week prior to the trip. Non-compliance may result in the cancellation of hotel reservations, rental car reservations, etc.

  • Transportation Van Reservation

    All SA van requests must be submitted five (5) business days before the reservation date or they will not be processed. Before submitting your request, please review the SA & RCCL Van Policy. If your request for the SA Van does not fit within the SA Van policy, please utilize the exception form.
