    Please coordinate with your other officers so that only one form is submitted per organization. Each student organization will be provided 1/2 of a 8 foot table, 2 chairs, and a sign for their table. Organizations with two or more affiliates will be given a full 8 foot table. No requests for additional table space will be granted. Set-up begins at 2:45pm.

  • Activity Reflection

    The Reflection Form serves to encourage student organizations to reflect honestly on their activities throughout the academic year. A thorough Reflection can provide invaluable direction and insight to future leadership of your organization and your advisors.
    In addition to reviewing for Quality, ARC uses the Reflections to see how involved your organization was on campus, whether the organization has learned from past experiences and implemented appropriate changes and how well attended events are in relation to projected attendance. The results of ARC’s review will be shared with SAAC during budgeting. It is imperative that a Reflection be completed honestly for each activity that your organization participates in throughout the year. Failure to complete a Reflection could negatively impact your organization’s status with ARC & SAAC for the following year.

    The College Supplemental Fund Application allows undergraduate students in AS&E to request supplemental funding as an individual or on behalf of a group to participate in academic, professional, or experiential programs. Funding is available on a rolling basis. Click here for more information.

  • SWARM Training Request

    Swarm Trainings need to be registered two weeks before the date you’re requesting. SWARM trainings are about 90 minutes in length. You need a MINIMUM of 5 people for a training, but you can have as many people as you want trained at the same time (example: you can have your whole chapter trained at the same time). Email ashley.toland@rochester.edu with questions or if you want to register a training for less than 5 people.

    This form is a way to share exciting news about you, a student organization, or another member of a student organization. Wilson Commons Student Activities will feature the exciting and noteworthy news through their outlets.

    This could happen in a number of places like the Student Organization Insider, in an email to the Deans or Communication.

    Some past examples are: On January 20th at Georgetown University, Mock Trial came in 4th out of 24 nationally ranked teams, with a record of 6-2. Jonathan Dietz & Hannah Brennan each won Outstanding Witness Awards. At ICCAs Central Semifinals on March 24th Keegan Cavanaugh-Stricker, member of Midnight Ramblers and UR Beatbox received Outstanding Vocal Percussion Award Society of Physics Students received a $500 Grant from National Office to host the Physics Pentathlon in April.

  • Community Kitchen Training Sign-Up

    Every reservation in the Community Kitchen is required to have at least one approved kitchen user at all times. Approved user status is valid for one year from the completetion of Community Kitchen Training. You can sign up for an upcoming training using this form.

  • Holiday Shopping Fair Vendor Application

    Our Holiday Shopping Fair is an annual tradition that WCSA has hosted for over ten years. Traditionally held at the begining of December, the fair is a way for students, faculty, and staff from across the University to interact and do some holiday shopping.

    This form should be used to report potential campaign violations. Not all reports will result in penalty points to candidates. You will receive a response to your report within 48 hours. Campaigning rules are available here.

  • Individual Travel Waiver and Release

    In order to travel, this form must be completed AND approved by 3:00 PM the business day prior to departure.

    For example, if traveling on a Saturday or Sunday, this form must be completed and approved by 3:00 PM Friday. Typical processing time is 1 business day.

    The Group Travel Form must be completed by the Trip Manager at least 1 week prior to the trip. Non-Compliance may result in the cancellation of hotel reservations, rental car reservations, etc.

    All SA van requests must be submitted five (5) business days before the reservation date or they will not be processed. Before submitting your request, please review the SA & RCCL van Policy. If your request for the SA Van does not fit within the SA Van policy, please utilize the exception form.
