Student Life Awards Nomination Form 2024-25
The annual University Student Life Awards recognize those students who—through service to others, investment of talent and time, and pursuit of excellence—have significantly and positively impacted the University of Rochester and/or surrounding community. The awards are divided into two categories: Individual Leadership Awards, and the Student Organization and Programming Awards. Award descriptions can be found here:

Instructions
Please complete the nomination form below and select the award from the Individual Awards list, the Group of Students or Organization Awards list OR the Student Organization and Programming Awards list. Only one name or one student organization per submission please. If you would like to submit multiple nominations, you MUST submit a new form for each nomination. Upon receiving the name of your nominee, an application packet will be forwarded to the student or organization for completion. Your nominee may also ask you to complete a recommendation form as part of their application packet.
Please note that while your submission is for a specific category, the Selection Committee may consider the student for one or more of the remaining awards if their application meets the respective criteria of each.
